Our Mission
We offer support to older adults and those that care for them by helping them find the best senior care options allowing them to live their best, most comfortable lives.
Grand Rapids Elderly Care, LLC was created out of a strong desire for helping adults with aging loved ones, and older adults themselves, to minimize the overwhelming nature of finding care- often with little time to do necessary research to find the right next place. Our goal is to help your loved one find the right place to live their best, most comfortable life and eliminate “I wish I would have known” from the equation.
We at Grand Rapids Elderly Care, LLC have toured most independent living, assisted living, memory care, and long-term care communities, and met with many in-home care companies in the Greater Grand Rapids area. We have made connections with the staff at each so that we can easily set up tours, understand availability, and make recommendations based on your personal, individual needs to ensure the best possible care for your loved one.
We understand the demands of raising a family, managing a career, and helping to care for an aging loved one. We also understand the struggles older adults face when they need to make care decisions for a spouse now facing a cognitive disease that makes caring for them alone at home now too difficult. Our commitment is to help find the next right place for you or your loved one.

Jen Koffman
Jen has always had a deep love for older adults. Growing up, she spent summers with her grandparents on their farm, and in high school, she became a “candy striper” in a long-term care facility. That early connection has continued throughout her life, including in her current role as a hospice volunteer in the Grand Rapids area.
Her passion for helping families navigate care decisions is deeply personal. After Jen’s mom passed away in her late 30s, she and her siblings faced difficult end-of-life decisions with little guidance. Years later, Jen found herself navigating another chapter of caregiving as her dad lived with dementia. Experiencing both journeys reinforced how overwhelming it can be to make important care decisions while balancing work, family, and the emotions of watching someone you love decline.
After 17 years in the corporate world, Jen felt called to combine her personal and professional experiences with her passion for helping others. That calling led her to create Grand Rapids Elderly Care.
As a Certified Senior Advisor, her mission is simple: to help families understand their options, find the right care, and feel confident in their decisions- so they can spend less time navigating the unknown and more time being present with the people they love. Most importantly, she hopes to help families avoid the words she knows all too well: “I wish I would have known.”

Senior Advisor:
Heather Johnson
Heather’s passion for working with older adults began at 18, when she answered a newspaper ad to help care for a 96-year-old gentleman living with dementia. Supporting him in remaining safely at home sparked a lifelong commitment to helping older adults live with dignity, independence, and quality of life.
For more than 30 years, Heather has dedicated her career to supporting older adults, including extensive experience as a Certified Occupational Therapy Assistant in subacute rehabilitation and long-term care. Her work has deepened her appreciation for each person’s story and individuality, while reinforcing her belief that everyone deserves to feel valued, respected, and cared for. With certifications in Aging in Place and Home Modification, she helps identify solutions that fit each person’s unique needs, whether at home or in a senior living community.
As a Senior Advisor with Grand Rapids Elderly Care, Heather’s approach is to listen first, understand what matters most, and walk alongside families as they consider their options. She is passionate about helping them feel less overwhelmed, better informed, and supported while connecting them with thoughtful solutions that bring confidence, clarity, and peace of mind.
